  1. The U.S. Postal Service requires all persons using a mail forwarding service to fill out USPS Form 1583. This is your authorization for us to receive mail in your own Personal Mail Box (PMB) at our office. Spouses may both use the same form 1583 if they have the same last name. An instruction sheet for this is included in the Mailing Sign Up Pack.

  2. Please fill out the two forms in the Mailing Sign Up Pack, "Application for Mailbox Rental & Mail Forwarding Service," and "Agreement to Forward Mail." The Application tells us who you are, such as name, location, phone, email, etc. Also, any special instructions from you regarding the handling of your mail.

  3. Enclose your check or money order with the application. Make sure the forms are signed and dated and just put them in an envelope and mail to us at:
    Your Best Address - 401 E. 8th St, Ste 214 - Sioux Falls, SD 57103.
Your Personal Mail Box number will be assigned the day we receive them, or you can call us on the phone and get your mail box number right away.

Call us at 1-800-419-1690 and we'll help you fill out the forms and answer your questions, or email us at: yba@midconetwork.com
